Certificate of Insurance Instructions

YMCAs attending the 2007 Great Lakes Zones meet need to provide a Certificate of Insurance to cover the YMCA of Central Ohio (plus two other rules acknowledgement forms).

Here is a Sample Certificate of Insurance naming the YMCA of Central Ohio. There are specific requirements for the Certificate of Insurance:

dates of coverage
liability amount
descriptions of operations
certificate holder.

You can include the Certificate of Insurance with the "Rules of Conduct" and "Rules of Time Standard Verification" you mail to Jon Reidler, or you can FAX or mail the Certificate of Insurance (COI) to:
